Cement Driveway Replacement

Looking for recommendations to rip up my cracked cement driveway, and replace it with a new cement driveway, divided up into about 10 small sections. Would also like the extension that goes from the sidewalk to the street also replaced if possible. Or is it required that the County replace the cement section at the end of the driveway that connects to the street? Anyone know the office that handles that in Fairfax County? In any case, I need the driveway replaced with new cement job. Would really appreciate recommendations from residents who have actually had similar work recently completed. Thanks. Approx 39 feet long and 17 feet wide.
